What is a Social Care Network?
The Social Care Network program helps reduce health disparities in low-income communities by using federal funding to connect Medicaid members with essential support—such as healthy meals, housing assistance, transportation, and other social services that influence overall health. By bringing social care and health care together, this program supports a major goal of the New York Health Equity Reform (NYHER) waiver amendment.

- Who qualifies? You may qualify if:
- Have active Medicaid
- Are part of a priority group: seniors, pregnant women, postpartum women (within 12 months), people with asthma, people with immune or chronic health conditions.

- Asthma & Air Quality Support: HEPA air purifiers, humidifier, dehumidifiers, air conditioners, HEPA Vacuum cleaner, allergen-reducing cleaning supplies, hypoallergenic bedding, pest remediation, and small-area mold remediation (under 10 sq. ft.)

- How do I get started?
- Fill out our short intake form (we only ask if you have Medicaid, your phone/email, and what time is best to call you).

- If you qualify, we schedule a home visit to provide services.

Yes. If you are a navigator, hospital, or community health partner, you can refer patients directly to us. We provide:- A fast intake process

- What if I don’t have Medicaid?
- This program is only available to Medicaid recipients under the 1115 Waiver. If you do not have Medicaid, we may be able to connect you with other community programs that can help.

- No. These services are an add-on benefit through the 1115 Waiver. They do not reduce or replace any of your existing Medicaid coverage.
